As a new patient, we ask that you please arrive 15 minutes prior to your scheduled appointment. For that first appointment, we will need your:

• Medical history
• Dental history
• Patient demographics
• Dental insurance

Please make sure to bring your current dental insurance card and a form of payment. We’ll see you soon!
